Twin A's tumour had already branched into multiple lineages, with different branches seeding different organs — a cancer that had developed in her for a long time. Twin B's skin and intracranial tumours contained only a subset of those branches, closely related to each other. If the twins had developed cancer independently, Twin B's tumour would not look like a clipped slice of Twin A's. The cleanest explanation: a single, relatively late in-utero transfer carried a batch of cancer cells wholesale into Twin B. At least two different cell branches were involved, so the transfer likely involved more than one tumour cell.

Mutations accumulate at a roughly steady rate during cell division, which gave a timeline. The gene fusion that drove the tumour formed around the end of the first trimester. The transfer to Twin B happened in the later part of the second trimester. The twins shared it, and its blood vessels connected them, allowing blood and cells to travel both ways. A striking side-finding supported this: about three-quarters of the cells in one spleen sample from Twin A carried Twin B's genetic markers. Twin B's blood cells had flooded Twin A's body. Note the direction of the two flows — blood mixing ran mostly B to A, while tumour spread ran A to B. Twins are a natural laboratory: they share genes and a uterine environment, yet develop separately.

The most systematic work comes from the Institute of Cancer Research in London. A study published in Leukemia followed nearly ten pairs of "discordant" identical twins — where only one twin had acute lymphoblastic leukaemia (ALL), the most common childhood cancer, about 80% of childhood leukaemias — for up to 15 years. When identical twins share a placenta, the healthy twin's risk was about 15% to 25%. For fraternal twins and other siblings, it was under 1%.

The study also supported the "two-hit" model: the first mutation happens in the womb and spreads to both fetuses through the shared placenta; the second happens after birth, likely triggered by a common childhood infection. Many children are born carrying tiny "pre-leukaemic" clones — most will never develop the disease. The practical advice that follows: if one twin is diagnosed, the other can be tested for pre-leukaemic cells every three months for the first year, then less often; risk is highest in the first 18 months after diagnosis, and screening usually stops after age ten. The first step, though, is confirming the placenta type — only about 60% of identical twins share one.

The rarer direction is mother-to-fetus. About one in a thousand pregnant women develops cancer, but documented transplacental spread to the fetus numbers only a few dozen cases worldwide — leukaemia, melanoma, lymphoma among them.
